The United States government forbids Makino and all United States companies from granting access to technical product data in the form of courseware to citizens or permanent residents of embargoed countries. Registrant understands he/she must confirm he/she is not a citizen nor permanent resident of any country on the United States embargoed or Denied and Restricted Parties List as it exists in fact.

Proper photo identification (i.e., driver's license or alternative photo ID) must be provided by all attendees to the instructor on the first day of training. Foreign national attendees must provide a passport identification prior to attending class. Makino reserves the right to refuse training or technology transfer to anyone who does not provide sufficient documentation. 

The information I will provide is accurate, true, and correct to the best of my knowledge. I understand and agree that my failure to provide accurate, true, and correct information or to respond to requests by Makino for additional information shall constitute grounds for rejection of my registration. I have read the above regulations and agree not to export or transfer the technology or documentation received in Makino's training classes.
